Eligibility and Redemption Details

Summit Broadband will cover up to $200 via a Visa Gift Card to eligible new residential customers who submit proof of an early termination fee (ETF) from their previous internet provider.

To qualify, customers must:

• Activate new Summit Broadband Internet service in a qualifying area
• Submit this form and a copy of their final bill showing ETF within 60 days of installation
• Remain an active customer for 60 days post-installation

Gift card will be issued via email or mail after successful verification. The email provided must match the email in your Summit Broadband account. Offer subject to change or end without notice. One redemption per household. Other restrictions may apply.

What Happens Next?

Once you submit your final bill and we confirm installation:

• We’ll review your submission within 10 business days
• If approved, we will email your electronic Visa Gift Card – up to $200
